Outbound: the agent calls

First-week check-in calls to new families

The first week decides whether a family stays. A friendly call after the third visit catches the PSW who arrives too early, the bath that should be in the evening, and the family who now wants overnights.

When it starts

A new client has had their first few visits, or a care plan just changed.

What the agent does

Calls the family contact, asks how it's going, records what's working and what isn't.

What your team gets

A note for the coordinator, with requests for more hours and any concern flagged.

How it works

  1. Picks the families

Each new client after the third visit, and anyone whose care plan changed in the last week.

  2. Calls the right person

The family contact on the file, at the time they said is best, or the client directly if they manage their own care.

  3. Asks plain questions

Are visits arriving when you expected? Is the help what Dad needs? Anything you'd change about the times?

  4. Hears the change

More hours, a different time, a second visit on weekends. Written down as a request for the coordinator, never promised on the call.

  5. Catches concerns

A PSW the client isn't comfortable with, a missed task, a new fall. Sent to the coordinator or nursing supervisor right away.

  6. Closes the loop

The coordinator sees every check-in on the client's file, and a family that's happy is noted too.

Example call

  1. AI agentHi Mr. Kowalski, it's Emma from Grand River Home Care. Your mom's had her first three visits with us. Do you have a minute to tell me how it's going?
  2. CustomerSure. The PSW's great, Mom likes her. But 7 a.m. is too early. Mom's not up yet.
  3. AI agentThat's helpful. Would 8:30 suit her better?
  4. Customer8:30 would be perfect. And honestly, we're thinking she needs someone at supper too.
  5. AI agentI'll pass both to Leanne: mornings moved to 8:30, and a supper visit added if a PSW is free. She'll call you tomorrow to confirm. Anything else on your mind?
  6. CustomerNo, that's it. Thanks for checking.

After the call: Leanne has a request on the client file: move mornings to 8:30 and add a supper visit, with "Mom likes her PSW" noted. Scheduling sees the open supper visit to fill.

What your team gets

  • A check-in on every new client's file after the first visits
  • Requests for more hours passed to the coordinator, ready to schedule
  • Visit time changes the family asked for
  • Concerns about a caregiver or the client's health flagged at once
  • A monthly view of what families said in their first week
Set up with you: The team sets up a connection to your AlayaCare, Procura or GoldCare client files and visit records, and sets with your care coordinators when the call goes out, what it asks and who gets each kind of answer.

Questions

Will it agree to more hours on the call?

No. It writes the request down and tells the family the coordinator will confirm. Hours depend on caregiver availability and the care plan.

What if the family raises a health concern?

It sends it to the nursing supervisor right away and tells the family to call 911 if someone is hurt or in danger.

Who does it call, the client or the family?

Whoever your file names as the contact. Many clients manage their own care, and it calls them directly.
